As of January 2010 we are in-network with insurance companies listed below. If you do not see your company, or have questions about network status, please check with your insurance. To check, call the number for customer services typically listed on the back of your insurance card. Medcost and PHCS are “umbrella insurances” and will have separate benefits for each plan. They should be able to let you know if we are “in network” or “out of network”.
NEW CHANGES: As of May 1st, 2011, we are NOT IN NETWORK with UNITED HEALTHCARE plans. Every policy differs in which services they will compensate for so make sure you check the website listed on the back of your insurance card or the use the 800/866 number to verify your benefits.
MEDICARE News: Currently we are not “accepting assignment” for new Medicare Patients. We do accept assignment for our current Medicare patients. If you would like to become a new patient that is covered under Medicare, full payment upfront will be required and we will file your insurance forms. If Medicare deems your visit “covered” they will reimburse the patient directly.
